5 Wishes Workshop: preparing a living will and advanced care plan

5 Wishes is a living will and advanced care plan that addresses personal, emotional and spiritual needs, along with medical and legal wishes when one is dying.  It is a legal document in the U.S. and in California only needs your signature and the signature of 2 witnesses.

5 Wishes addresses and designates

  • who you want to make health care decisions for (your legal Power of Attorney / health care agent)
  • the kind of medical treatment you do and don’t want (medication, life support) in different situations (close to death, in a coma, brain damage, etc.)
  • how comfortable you want to be (pain management, physical contact, hygiene, etc.)
  • how you want people to treat you (where you want to be, who you want to be with, what you want them to be doing, or not doing)
  • what you want your loved ones to know

It’s also a way to initiate conversations about death with your loved ones.

During the month of October we will be discussing impermanence and death on Sangha Nights. We’re also offering this 3 hour workshop to complete the legal 5 Wishes Document. We’ll go through it together, step by step. You can then choose to keep yours as a hard copy, or it can be a rough draft if you decide to do the online digital option, that you can easily change at any time.

* We would love everyone to attend in person, but we understand not everyone is able to do this yet, so if you need a zoom option please contact us and we will set that up.
